The following are seen as the major barriers to embedding quality into any organisation and enabling continuous improvement:

- emphasis on quick fixes and low-level reforms, short-term performance at the expense of long-term improvements;
- insufficient employee empowerment;
- lack of cost-of-quality measurement, performance reporting, and reward / formal recognition systems;
- lack of cross-functional, cross-disciplinary efforts;
- lack of focus in training and coaching;
- lack of long-term commitment and leadership for management;
- misdirected focus - emphasis on the trivial , addressing the many problems facing your company rather than concentrating on the critical few;
